WHAT: Signature Dental Gold Plan is an annual reduced fee dental plan that allows individuals and families -- just like yours -- to receive quality dental services, through your employer, from Signature Dental at prices that make sense for today’s economy.  It is a discount dental plan and cannot be used in conjunction with any other discount plans or insurance programs. Signature Dental Gold plan offers the economic benefits of large group dental care, but it is delivered in the personalized setting of a private practice.  This discount is available for employers looking to add employee benefits to their compensation packages.  Because we are a private practice, we are limited in the number of members we can see without becoming a large impersonal volume based office.  With this in mind, should there be a waiting list, you will be contacted as soon as space becomes available.  Please contact our office at 407-833-8777 to see if there is any availability for new enrollment in the Signature Dental Gold Plan.

HOW: After your employee applications have been processed, accepted, and your membership has been made active, simply call 407-833-8777 to schedule your new patient exam.  This discount plan is a stand alone plan only and can not be used in conjunction with any other dental plans that you may have. At your first visit you will be issued a membership card which demonstrates your current eligibility to our office and those of our participating specialists.

WHO’S ELIGIBLE: All employees who wish to enroll with our program. The worker and their spouse are eligible, as well as their children under age 19 or full-time students up to 23 years of age. Coverage of a child who attains age 19 will be continued while incapable of self sustaining employment by reason of developmental disability or physical handicap. Children may be members only as dependents of adult members.

WHEN: Enrollment is effective immediately with no wait periods.   If an employee is terminated or ceases employment with the enrolled company, their benefits in the Signature Dental Gold plan are terminated immediately. 

PAYMENTS: All payments listed under the Reduced Fee Schedule in your enrollment brochure are made directly to the dental office, in full, at the time the treatment has been rendered.  There are no deductibles to be met, nor are there restrictive annual maximum benefit allowances.  Signature Dental does reserve the right to revoke your membership in this plan at any time.  Membership in this plan is good on an annual basis.  Renewal into this program and the discount availability will be determined by Signature Dental.  Membership in this program does not carry an annual fee for you and your family, however, Signature Dental does reserve the right to later add annual fees, and or limit the number of participants in this program.
