Signature Smiles for Life

Every patient in our office deserves the very best, that’s why we’ve decided to offer the Signature Smiles for Life program!

 

When you come to our office for your prevention examination, x-rays and cleaning (D0150, D0330, D0120, D1110 or D4910)*, we will provide you with a set of custom tooth whitening trays and materials for a one-time enrollment fee of only $99.  Then, at each recommended preventative visit, we will give you a complimentary touch up vial of bleaching gel! (a maximum of 2 vials can be distributed yearly)  This program will ensure that you stay current with your preventative exams and maintain that beautifully healthy, vibrantly white smile for the rest of your life!

 

What we ask of you in return is:

 

•  You keep and maintain your doctor recommended preventative visits.  Your long term dental health is important to us, so much that we’re giving away our whitening gel to help motivate you to stay current with your prevention schedule.  This preventative strategy has helped patients reduce their incidence of emergencies.

 

• You must provide 48 hours of notice if you need to change and reschedule a preventative appointment.  In order to provide the exceptional services like Signature Smiles for Life to all of our patients we ask that you give us the courtesy of advance notice.

 

•  If you have failed to uphold to one or both of these requirements you can re-enroll into the Signature Smiles for Life program for only $99.
